Submission Instructions
*Email your special request:
*Type, "Birth Certificate" in Subject line
*Specify your Certificate theme (found under each cert.): Example: butterfly.
*Type the EXACT name and Birthdate; following the basic certificate formats above.
*Make sure your Email address is correct & deliverable.
